Why experienced case managers are joining Breakthrough

With years of experience, award-winning case manager, Holly Luxton, found herself missing the collaboration of a team. Now, she’s part of a forward-thinking company that blends case management with expert in-house care, creating a truly joined-up approach to rehabilitation.

We caught up with Holly to explore why she made the move and how Breakthrough’s culture of support, innovation, and teamwork is shaping her career…

Q: Holly, you’ve had a diverse career in case management. What led you to join Breakthrough Case Management as an associate?

Holly Luxton: I’ve been a case manager since 2017, working with large organisations and later as an independent case manager.

While working solo had its benefits, I missed the support and collaboration of a team.

When Breakthrough approached me, I was intrigued by their business model that integrates case management with in-house care provision.

Having expert care advice on hand was a game changer—it truly enhances the client experience and creates a more joined-up approach to rehabilitation.

Q: What was it about the team at Breakthrough that appealed to you?

Holly: Case management is inherently an autonomous role, but that doesn’t mean you have to feel like a lone wolf.

At Breakthrough, I was drawn to the idea of working alongside a team of case managers with diverse clinical backgrounds.

Having a wealth of expertise to tap into, share knowledge with, and collaborate on complex cases makes a huge difference. It’s a supportive, team-oriented environment where everyone contributes.

Q: Breakthrough is known for being forward-thinking. How has that impacted your role?

Holly: Absolutely! It felt like an exciting time to join the company.

There’s a real commitment to innovation at Brealkthrough, and that creates fantastic opportunities for personal and professional growth.

In fact, I’m now helping to develop a new service provision that’s in the pipeline, which is both exciting and rewarding.

Q: How has Breakthrough’s leadership and company culture influenced your experience?

Holly: From day one, I felt valued and supported. There’s an open-door policy from the top down, which fosters a real sense of inclusion.

You’re encouraged to share ideas, ask questions, and seek guidance without hesitation.

That level of accessibility and support makes a huge difference in continuing to feel confident and empowered in your role.

We have a monthly case management meeting, often with guest speakers, and a lively Teams channel where we can ask our colleagues for their advice or just check in from time to time.

Q: What would you say to other experienced case managers considering Breakthrough?

Holly: If you’re looking for a company that truly supports its team, fosters collaboration, and provides growth opportunities, Breakthrough is the place to be.

The training and learning opportunities are fantastic, and the team culture spreads the weight of responsibilities, so no one feels isolated.

It’s a great balance of autonomy and support, making it an ideal environment for any experienced case manager looking for their next step.
