Patient support service Onward has joined with Active Care Group to provide clinical advice and support to patients with life-changing injuries and illnesses.
The case management division of Active Care Group has partnered with Onward, a Community Interest Company established to provide free clinical and care advice and support for patients and their families in hospitals across England.
Led by Stewarts Law and together with its other partners – University Hospital Southampton NHS Foundation Trust and Wessex Trauma Network – Onward works in hospital settings to enable families can access a variety of free support services, including financial, emotional and legal advice.
Keith Browner, chief executive at Active Care Group, said: “We are excited to be chosen as Onward’s official care partner of choice.
“The new partnership will allow our case management division team to expand their extensive network and reach more people in need of our services.”
Managing director of Active Care Group’s case management division, Janette Wynn, said: “We are thrilled to be working in partnership with Onward, the team at University Hospital Southampton NHS Foundation Trust and the Wessex Trauma Network. This is an amazing opportunity to ensure that patients and families receive all the elemental support they would need in addition to their hospital care and look to receive the best possible outcomes for all.
