Passion, experience and the power of connection in case management

When you sit down for a chat with Kate Lewis, her passion for case management is immediately clear.

With 16 years of experience and a reputation that precedes her in the industry, Kate is not just a case manager—she’s a mentor, an advocate, and a leader in her field.

Since joining Breakthrough Case Management last year, she’s been thriving in an environment that aligns with her values and allows her to make a real difference.

“I was drawn to Breakthrough because it felt dynamic, exciting, and personal,” Kate explains.

“I didn’t want to be just a number in a great big company. I wanted to be part of something where my contributions were valued, where I could bring ideas to the table, and where I felt truly included.”

One thing that stands out when talking to Kate is her deep commitment to relationships—whether with her clients, colleagues, or fellow case managers.

“Case management is all about connection,” she says.

“It’s not just about supporting clients; it’s about fostering strong relationships within the team, exchanging expertise, and learning from each other.”

Kate’s extensive experience has led her to work with a diverse range of clients, and she takes great pride in the longevity of those relationships.

One client in particular, now in his early 20s, has been with her since he was just nine years old.

“The fact that my clients have chosen to stay with me as I joined Breakthrough means everything,” she says.

“It’s a testament to the trust we’ve built over the years.”

Beyond her role as a case manager, Kate is also dedicated to supporting the next generation of professionals.

As an assessor of advanced practitioners at BABICM, she is passionate about passing on her knowledge and helping others excel in the field.

“I love being able to mentor and guide new case managers,” she shares.

“Breakthrough’s approach of valuing each team member’s background and expertise makes it the perfect place to do that.”

For Kate, joining Breakthrough was not just a career move—it was the right step at the right time.

“It was an opportunity to be part of a company’s evolution, to help to shape its future while continuing to grow myself,” she reflects.

“I love the autonomy I have in my role, but I also need that sense of being part of a team. Breakthrough has given me both.”

As we wrap up our conversation, Kate laughs and glances at her watch.

“I’d love to chat more, but I’ve got to dash—I’m off to a solicitors’ Bollywood fundraising evening! One of my clients has even helped me choose the perfect Indian outfit for the occasion.”

That moment sums Kate up perfectly—deeply engaged, relationship-focused, and always embracing new experiences.

Her dedication to case management, her clients, and her colleagues makes her an invaluable part of Breakthrough, and we are proud to have her on board.
